Volunteer Application 2008

Registration Form

All players must have the following form filled out and turned into their coach at the beginning of every season:
• BELL Medical Release Form (pdf)

If a player sustains an injury at a practice or game, these are the forms coaches may need:
• Accident Notification Form AIG (pdf)
• Claim Form Instructions AIG (pdf)
• Injury Tracking (pdf)
• What Parents Should Know About Little League Insurance (pdf)

Pitching forms:

• Pitching log
• Pitching Eligibility Form

All-Star forms:

• All-Star Eligibility Form (pdf)
• All-Star Commitment Form (doc)
• Washington State Birth Certificate request site (All players must provide an original copy of their birth certifcate.)
